Purpose: To gather information daily on specific workshop
activities
Administered To: Faculty members who teach engineering
to undergraduates
Topics Covered:
- Workshop Evaluation: content, materials, methods,
objectives, organization, practical value
Format/Length: 23 questions total, 18 closed-ended
Likert scale and 5 open-ended spaces for comments. This form
was used for the activities conducted on Monday of the 5-day
workshop. (There are similar sets of questions for Tuesday through
Friday activities, but because the questions for each day are
very similar, only Monday's questions are included.)
