Purpose: To collect background data from particpating faculty and their assessment of program implemention and training activities

ratings of the effectiveness of the project along a number of dimensions (e.g., changing existing curricula)
evaluation of workshops and retreats

Format/Length: Mix of 24 closed- and open-ended questions; space for teachers' general comments.
