Instructions for Evaluating Students Enrolled in
AQS 111 Practicum II

The evaluation process should be completed during the last week of the practicum. Upon completion, return the evaluation to: Aquarium Science Program, Oregon Coast Community College, 332 SW Coast Hwy, Newport, OR 97365.

The evaluation is a three step process and includes:

  1. Completing the skills evaluation form.
  2. Completing a written narrative by the practicum supervisor and the student.
  3. Discussing the evaluation with the student.
  1. The skills evaluation form contains numerous skills and work performance criteria. The person who had the closest interaction with the student (i.e. the supervisor) should complete the form. If several individuals worked closely with the intern, then a group evaluation process is acceptable. Simply mark the column that most appropriately describes the intern’s performance for each item.

  2. The written narrative should include your overall impressions and observations of the student’s work performance. This portion should include specific recommendations on how the student can enhance their performance and as well as how they can continue to develop professionally. Also note the individual’s contributions to your operations. Note whether the student performance has developed or regressed during the practicum.

  3. The discussion with the student about their performance should occur during the last week of the practicum. The meeting should be scheduled in advance and held in a private setting to minimize interruptions and allow for open discussion. Once signed, the student is entitled to a photocopy of the documents if they so desire.
