The Collaboratives for Excellence in Teacher Preparation (CETP) is a major national effort by the National Science Foundation to significantly improve the mathematics, technology, and science education of prospective elementary and secondary teachers. This program supports large-scale systemic projects designed to change teacher preparation programs on a state or regional basis and to serve as comprehensive national models. Many of these projects involve multiple institutions of higher education within a state or regional area. You have been identified as a faculty member at one of these participating institutions of higher education who has been involved in the collaborative project effort on your specific campus.

DIRECTIONS

  • The term "collaborative" is used throughout the questionnaire. When you see the term "collaborative," please think about the program on this campus and/or the project statewide, whichever is most relevant to a particular item.

  • Please read all questions and instructions carefully. Answer the question by circling the appropriate number or writing in the answer, if requested to do so. It is important that you limit your response to one alternative for questions that direct you to "CIRCLE ONE." We cannot use your answer if it is between two responses or if you change the wording of the question in any way.

  • When you have completed the questionnaire, please return it to SRI International in the enclosed envelope.

    1. What has been your level of involvement in each of the following collaborative activities? If the activity is not part of your collaborative or if the activity has not yet been implemented on your campus, circle "9" on the appropriate line. (CIRCLE ONE NUMBER ON EACH LINE)
