  • Course Evaluation: satisfaction, effectiveness, ease of use, technology use, engagement, importance, areas for program improvement
  • Demographics (Student): gender, ethnicity, age

Format/Length: 23 scaled-items, 1 yes/no item, and 6 open-ended items
