Step 1:. Determine the best delivery mode for the questionnaire.
Step 2: Decide if a special incentive should be offered
for responding to the questionnaire.
Step 3: When possible, assure confidentiality to each participant,
explaining the system by which this is accomplished.
Step 4: Notify each participant ahead of time that he or
she will be receiving a questionnaire, explaining its purpose and
importance.
Step 5: Attach a cover letter to the questionnaire that
reiterates the purpose and the importance of the questionnaire and
describes the incentives and confidentiality policies you have established.
Step 6: For questionnaires administered off-site, be prepared
to follow up with participants who do not respond.
Step 7: Check all questionnaire responses for completeness
and interpretability before data entry and analysis.
