Questions I.3, I.4, and II.1-9, need to use radio button formats. Question II.10 needs to use two separate text boxes. Other modifications should include shortening text lines, adding navigational cues specific to computer use, and deciding how respondents will move from one page of the survey to the next (it is unlikely the survey could fit on one computer screen). After these changes, it is critical to test if the survey appears correctly on different kinds of computer systems.
