Question 1 assigned a high number to the most positive score. This question does the opposite.
Choices 2 and 3 need headings.
The format does not allow the respondents to explain their opinions. You might add an open-response question to ask them to explain the reasons for their rating.
It would be a good idea to break up this question into a set of questions, each focused on a different component of the workshop, and using the same response scale.
